Please try to arrange to arrive 15 minutes before your first appointment time to allow adequate time to fill out our Patient Registration and New Patient History. The Patient Registration form can be printed at home so that you can bring it to your appointment already filled out if you would like. You can find it on the Forms and Resources page of our website under Forms.
Please allow for 1-2 hours for your first appointment. This will allow for any kind of testing necessary as well as time for Dr. Hassett to explain everything you need to know and also to ask any questions that you have. As a new patient, it is likely that there could be some allergy testing. Please read the section below about the prerequisites to testing.
We accept most insurance plans. However, if your health insurance requires a referral to see a specialist, it is necessary that you contact your primary care physician and obtain that referral prior to your visit. We will need to have your referral information and insurance card on the day of your first appointment. If you have any questions about this policy or are unsure if you require a referral, feel free to call our business office at 360-260-0502 or call the number on the back of your insurance card and speak to your insurance company.

As part of your allergy evaluation, skin testing may be recommended. Antihistamines (a common ingredient in allergy or cold medicines) can block your skin's reactivity to testing making them less accurate. Please do not take medicines such as: Allegra (fexofenadine), Claritin or Alavert (Loratadine), Clarinex (desloratidine), Zyrtec (cetirizine), Xyzal (levocertizine), Benadryl, Chlor-Trimeton, Actifed, Nyquil, Dayquil, or generally any medications labeled for allergies or colds. Nasal steroid sprays such as Flonase, Nasonex, Nasacort, Veramyst, etc are fine to continue. If you have a question about a specific medication feel free to call our office and ask one of our nurses.
Other medication prescribed by your physician such as blood pressure, thyroid replacement, cholesterol medications, diabetic medications, antidepressants etc, are fine and should be continued.
If you have been diagnosed with allergies and have decided you would like to treat the problem at the immune level with allergy shots rather than blocking with medicines, or if you are already established on allergy shots: You can receive your allergy shots at our offices. We have competitive shot clinic hours and convenient locations.
